Insurance Information

We are contracted with most major insurers and are always striving to add more. Please contact your insurance company to verify if we are a contracted provider with your insurance. If your insurance is not listed, you can still see us and may qualify for a self-pay discounted rate

We advise that all patients verify that your Care Center is an in-network provider for your insurance. Also, check which lab your insurance company is contracted with. You are responsible for any fees that are not paid by your insurance carrier.

Newborn Notice: Prior to your first appointment, please be sure that you have contacted your insurance company to add your new baby to the insurance policy within the first 30 days of life. Make sure to bring your insurance card with you to your first appointment so we have everything we need to submit your baby’s claims after you have added your baby to your plan. If your claim is denied, you will be responsible for payment.

Insurance Coverage and Patient Responsibility Disclaimer

As the patient or patient’s guardian, you are responsible for providing accurate and up-to-date insurance information at each visit. It is your responsibility to understand your insurance coverage, including copayment amounts, coinsurance amounts, and deductibles. Furthermore, you must be willing to commit to paying all copayments, coinsurance, and deductible amounts as determined by your insurance carrier.

We, acting as a third party, cannot engage in prolonged insurance negotiations, and you will need to accept this as your responsibility. As a clinic, we do not have any input in determining your benefits or coverage provided by your insurance plan. If you believe there is an error in the amount covered by your insurance plan, you must contact your insurance carrier directly. In the event that you are no longer eligible on the insurance plan you provide, you will be held liable for the full amount of any outstanding balances.
